Summer Camp Forms

These forms are required to attend camp.

Forms – Troop

Attendance Roster/Ship Manifest (fillable): Each troop should have this form filled out as they board the boat to arrive indicating how many scouts and adults are coming.
AB 506 Compliance Form (fillable) For California Troops Only

Forms – Adult

Annual Health and Medical Record ABC: ALL Participants must complete this form within 12 months of arrival at camp.
Pre-Camp Release Form – Adult: : Provides an overview of program offerings and rules as well as an exculpation and indemnity provision.

Forms – Youth

CA Firearms Release Form x2 copies (1 for Camp Office, 1 for Range) Required for youth who want to shoot Rifles and Shotguns at camp.

Mandatory Pre-Summer Camp Forms

Pre-Summer Camp Release Forms – Adult

Pre-Camp Release Form – Adult: : Provides an overview of program offerings and rules as well as an exculpation and indemnity provision. Please Note: All adults coming to camp must be registered in the BSA, complete Youth Protection Training, and have their A, B, and C portions of the Medical Form filled out.
**Adults with ASA 101, 102, 103, or 110 Sailing Certifications must bring their card if interested in chartering one of the Beneateau 22 sailboats.

Pre-Summer Camp Release Forms – Youth

Pre-Camp Release Form – Youth: Permission form required for all youth
California Rifle and Shotgun Parental/Legal Guardian Permission Form: Provides a firearms permission and release. Required for Rifle and Shotgun. Please arrive with three forms printed: one for the Clerk’s Office to email a record to the Council, one for the Range Safety Officer, and one to be on the Scout whenever shooting.

2026 Spring Leaders Guide: Information discussed at the Spring Leader’s meetings is contained in this packet.
2026 Merit Badge Scheduling Worksheet: This form is for Scouts to use when figuring out their merit badge schedule for camp. They will turn it in to their Scoutmasters for registration.

Merit Badge Sign-up Instructions: This PDF will walk you through the steps to sign up for Merit Badges at Camp Emerald Bay.
2026 Merit Badge Prerequisite Form should be used for the Merit Badges requiring work outside of the camp provided time.

2026 Program Planner: The new edition of the Program Planner has a breakdown of the week and its events. This is subject to change once the CA COVID Guidelines are released. In addition to the Planner, we have created an Additional Activities Form that gives more insight on programs and activities throughout the week and for adults in camp.
USS Iowa Flyer: Stay overnight at a discounted rate to be closer to the ferry terminal in the morning.

Additional Forms

2026 Pre-Order Camper T-Shirt. Pre-ordering saves you $5 per shirt. All pre-orders need to be submitted by May 30th, so we have them ready for you during your week in camp. Recommended to purchase by unit, but individual purchase is okay.
2026 Online Store now open for pre-camp orders and in-camp pick up. All orders need to be submitted by May 30th to qualify for in-camp pick up.
2026 Emerald Bay Special Requests. This online form is used for units who have special needs for the summer.
2026 Emerald Bay Boat Transportation Departure Schedule
Campership Request Form:  This application puts scouts in the Western Los Angeles County Council into consideration for financial assistance for their trip to camp.
Reservation Change Request Form: Use this form to reduce or increase the number of participants you will have attending.
Child Care Exemption. We receive many requests for the Council’s Tax ID number related to camping.

SCUBA Required Forms

Camp Emerald Bay SCUBA Registration 2026

All participants are required to have a doctor’s authorization on their PADI RSTC Medical Form, regardless of the answers on the form and what the form says. Please note that the medical team at camp have final say on medical forms and diving. We recommend you view the diving requirements for Scouts prior to signing up by going to https://www.scouting.org/health-and-safety/gss/gss02/ and scroll to the BSA Scuba Policy section.

The following forms are required for all people participating in a SCUBA program at Emerald Bay. Please bring these to camp signed and ready to turn in. Thank you!
